Site Establishments: NHCA played a crucial role in selecting optimal locations for two SUR Northland sites, including a primary care clinic in Clay County and a Prenatal & Pediatric Clinic in Creekwood Commons.

Healthcare Impact: Since 2010, the Clay County clinic facilitated over 42,000 patient visits, while the Prenatal & Pediatric Clinic supports low-income pregnant women through the Healthy Families program.

Healthy Families Program: Launched in 2021, this NHCA Care Coordination program offers comprehensive care coordination, including Medicaid and WIC enrollment, primary care, oral health, and behavioral health services, focusing on vulnerable populations in the Northland community.
