General Questions

Northland Health Care Access (NHCA) is not a Health Insurance Policy, we are a non-profit organization with the goal of enhancing access to quality healthcare services for all Northland residents.
NHCA partners with hospitals systems, providers and other healthcare organizations throughout the Northland who donate care to our patients. Through NHCA Care Coordination, patients in the programs are set up with appointments and follow-up appointments, including transportation to and from, when needed. Through Care Coordination, patients are provided with vital services like prescription assistance, medical and health supplies ranging from bandages and diapers to soap and tissue available for pick up or delivered to the patient’s home.
Patients with health insurance but who are under insured may still qualify for our Care Coordination program! We work with individuals who have limited financial resources (at or below 400% below Federal Poverty Level (FPL)). While these individuals are eligible to purchase private health insurance on the Affordable Care Act (ACA) Insurance marketplace, or may have Medicaid coverage, paying for the co-pays and their medication prescriptions create a financial hardship. Patients must be 18 years or older. Patient’s income must not exceed 400% of the FPL. Patients must be a Clay, Platte or Ray County resident although we are able to assist, in some capacity, residents in surrounding counties. View our programs to check the in-depth qualifications.

Programs and Services

NHCA offers several programs, each with different services ranging from pregnancy support to older adult care. View our Programs tabs for more information about each program.
This is a continual program for our patients, until our services are no longer needed, so long as the patient qualifies. Our patients renew yearly, and the patient budget changes yearly. Patients have access year-round to our services, but funding does not carry over to the following year.

Older Adults FAQs

No, a requirement is that all older adults in the program are independent.
60+ age is required to participate in the program.

Healthy Families FAQs

Second or third trimester is best to pick up baby/mother supplies.
o Yes, baby items and mother postpartum supplies will be available, including breastfeeding supplies and resources.

NorthlandCARE FAQs

No, under the NorthlandCARE program, there is no patient Co-pay. Basic Lab work through our 3 partner laboratories is also included under the NorthlandCARE program. This does not cover referrals sent by the provider to specialists.

NorthlandCARE program does not cover the cost of prescriptions, but we do have a limited budget per patient to help with prescription costs. We also exhaust resources for financial assistance through coupons and manufacturer discounts or assistance.

NorthlandCARE Plus FAQs

NorthlandCARE Plus program does not cover the cost of patient care, however, we will reimburse patients for co-pays, medical bills and prescription receipts.
Through the NorthlandCARE Plus program, our Care Coordinators are able to assist with the purchase of medical necessities and supplies available for pick up or even delivery.

Continuous Glucose Monitoring (CGM) FAQs

No, the cost of the service for CGM is covered by the CGM program in NHCA.
